Admission Procedures

 

For Freshmen

Step 1:

Submit the following requirements to the Registrar’s Office:

a. Entrance examination result (CEE result);

b. Original copy of the following:

– Certificate of Good Moral Character;

– Form 138 or 4th year high school card;

– 1 copy of 2×2 I.D. picture;

– Police Clearance

– Certification from the Medical & Dental Clinic;

– Chest X-ray result (negative)

Step 2:

Pay the assessed amount at the Cashier’s Office.

Step 3:

Go to the NSTP Office to register your name in any of the three components of the program.

Step 4:

Sign Student’s Pledge at the Office of the Student Services.

 

For Old Students

Step 1:

Get an Assessment Slip at the Accounting Office.

Step 2:

Pay the assessed amount at the Cashier’s Office.

Step 3:

Wait for your Registration Form.
